Role summary
[seed_staging_v1] Hospital pharmacy placement covering ward-based clinical services, medicines reconciliation and patient counselling within a multidisciplinary team.
Key responsibilities
- Provide ward-based clinical pharmacy services
- Conduct medication reviews and reconciliation on admission
- Deliver medicines information and patient counselling
- Contribute to medication safety and quality improvement work
Essential requirements
- Pharmacy degree and eligibility for GPhC registration
- Hospital or clinical pharmacy experience
- At least one year of post-qualification practice
- Strong communication and interpersonal skills
Desirable requirements
- Postgraduate clinical diploma
- Antimicrobial stewardship experience
